Rhytidectomy (facelift)
Rhytidectomy, commonly known as a facelift, is a comprehensive surgical procedure that removes excess skin and tightens the underlying structures of the face and neck. Who is a facelift suitable for
Rhytidectomy is primarily intended for patients aged 50 and above who are dealing with significant sagging of the facial and neck skin, deep nasolabial folds, drooping cheeks, or a double chin caused by fat accumulation and laxity of the neck muscles. The procedure also addresses tissue descent in the cheek area and lower third of the face that results from natural ageing, gravity, and loss of subcutaneous fat. The ideal candidate is a non-smoker in generally good health with realistic expectations. A facelift is not a substitute for a healthy lifestyle, but rather a means to restore a natural, rested and fresh appearance that matches how the patient feels on the inside. MACS method and SMAS technique
At Aesthgery clinic we employ two well-established surgical techniques. The MACS method (Minimal Access Cranial Suspension) is a less invasive approach in which the surgeon introduces special absorbable sutures through a minimal incision. These sutures lift the sagging tissues of the cheeks and lower face in a cranial direction and fix them in their new position. The result is natural tightening without excessive skin tension. The SMAS technique (Superficial Musculo-Aponeurotic System) goes deeper — it works with the musculo-aponeurotic fat layer beneath the skin, which is the key structure for achieving a long-lasting lifting effect. By tensioning and repositioning the SMAS layer, we achieve a more stable result because the skin is not subjected to excessive tension and scars are less noticeable. The choice of technique depends on individual anatomical proportions and the extent of correction required, and is always discussed during the pre-operative consultation.
The procedure and incision placement
Rhytidectomy is performed under general anesthesia and takes 3–4 hours. The operation begins with an incision in front of the ear, continuing around its lower part and behind the ear into the hairline. This incision path allows excellent concealment of scars within physiological folds and the hair. In patients with pronounced neck concerns, a small incision is added under the chin. The surgeon accesses the SMAS layer, adjusts it, removes excess fat from the double-chin and neck area (platysmaplasty), and then trims and removes the excess skin. The resulting scars are virtually invisible after 6–12 months. After the procedure, a compressive bandage is applied and the patient is hospitalised for 1–2 days for safe recovery initiation under professional supervision.
Recovery, risks and longevity of results
Immediately after surgery, expect swelling, bruising and temporary skin sensitivity, which are part of normal healing. A compressive bandage or elastic mask is worn for approximately one week. Sutures are removed on day 7–10. Most patients return to normal social life within 2–3 weeks; physical activity and sport are permitted after 4–6 weeks. The final result, once all swelling has resolved and scars have matured, is fully visible after 3–6 months. The results of rhytidectomy are long-lasting. Possible risks include temporary swelling, bruising, temporary loss of sensation, or asymmetry.
