Breast Augmentation
Breast augmentation is the most commonly performed plastic surgery in the world. Using silicone or saline implants, it enlarges and shapes breasts according to the patient's wishes. At Aesthgery clinic in Pilsen, every augmentation is preceded by a thorough consultation during which the surgeon and patient together select the optimal implant size, type and placement to achieve a natural result that is harmonious with the overall figure.
Procedure overview
Implant types and consultation
A detailed consultation is essential before surgery to determine ideal implant parameters. The most widely used are cohesive silicone gel implants — their consistency resembles natural breast tissue and even if the shell is damaged, the gel does not leak. Implants are available in round or anatomical (teardrop) shapes and various projections. Saline implants filled with salt solution are less common and are typically chosen for specific indications. During consultation, the surgeon considers the starting shape and volume of the breasts, chest width, existing skin and tissue. The surgeon helps with size selection through sample sizers, result photos and computer simulation to ensure the outcome matches the patient's vision and natural proportions.
The procedure and approaches
The surgery is performed under general anesthesia and takes approximately 1–2 hours. The surgeon selects the implant insertion site (approach) and the placement plane. The most common approach is the inframammary fold incision, which provides the best visibility and minimizes risks. The periareolar approach (incision around the lower half of the areola) leaves a scar hidden at the skin-areola border. The axillary approach (through the armpit) leaves no scar on the breast but is technically more demanding. The implant can be placed in the submuscular plane (under the pectoralis major muscle) — delivering a more natural result and reducing rippling risk in slim patients — or in the subglandular plane (above the muscle, under glandular tissue), which is less painful with a shorter recovery. The surgeon recommends the optimal combination based on individual anatomy.
Recovery after augmentation
After surgery, the patient stays at the clinic for 1 day. A compression bra or elastic bandage is worn continuously for 8 weeks and is essential for proper healing and implant settling. Sutures are absorbable; the ends are trimmed at approximately 3 weeks. Swelling, tightness and breast sensitivity in the first weeks are normal and gradually subside. Physical activity — especially exercises involving chest muscles and lifting heavy objects — is restricted for 4–6 weeks. Most patients return to work in approximately 1–2 weeks. Implants settle into their final position during the first weeks and breast shape gradually stabilizes. The final result is fully visible after 2–6 months.
Risks, implant lifespan and results
Breast augmentation is a safe surgery, yet carries certain risks like any surgical procedure. Possible complications include infection, capsular contracture (hardening of scar tissue around the implant), implant displacement, sensitivity changes, hematoma or seroma, and rarely gel bleed. At Aesthgery clinic, only certified implants from leading manufacturers with proven safety and longevity are used. Replacement is not necessary unless complications are present or the patient wishes a size change. Augmentation results are long-lasting and deliver significant improvement in symmetry, shape and confidence. The right choice of implant size and type is key to a natural result that harmonizes with the overall figure and the patient's wishes.
