What is gynecomastia and who is affected
Gynecomastia is the benign enlargement of glandular breast tissue in men. It affects men of various ages — it may appear during puberty (often temporary), in adulthood, or in older age. Causes include hormonal imbalance (estrogen-to-testosterone ratio), use of certain medications, anabolic steroids, being overweight, or idiopathic origin. Gynecomastia must be distinguished from pseudogynecomastia, which involves fat accumulation only, without a glandular component. We clarify the diagnosis at consultation through physical examination and, if needed, ultrasound. Surgical treatment is indicated for men where conservative approaches (medication adjustment, weight reduction) have not yielded results, or for established fibrotic gynecomastia.
The procedure
The surgery is performed under general anesthesia and takes approximately 1–2 hours. The surgeon combines two techniques: tumescent liposuction, which removes excess fat from the breast area and surroundings, and subcutaneous mastectomy through a periareolar incision (placed along the edge of the areola), through which the glandular tissue is removed. This approach minimizes the extent of scarring and preserves the natural chest shape. In cases of purely fatty gynecomastia (pseudogynecomastia), liposuction alone without direct excision may be sufficient. After surgery, a compression garment or elastic bandage is applied.
Recovery and results
After surgery, drainage tubes are placed and removed after approximately 5 days. Wearing a compression vest or elastic bandage for 8 weeks is essential so that tissues heal correctly, swelling subsides, and the final shape settles. Swelling and bruising resolve within 2–3 weeks. Patients in desk jobs typically return to work within 2–3 weeks. Sport and physical labor are possible after 4–6 weeks. Results are permanent — removed glandular tissue does not regenerate. The chest takes on a flat, naturally masculine shape. Scars from the periareolar approach are discreet and, after healing, blend with the areola pigmentation.
